How to Find Healthcare

Below are some tips on how to find healthcare near our campus. We’re providing this information as a service to our students, but you may find other locations and providers through online search. Ultimately, the choice of provider is determined by the student and/or the student and their family.

Pharmacy
Hartford HealthCare Medical Group has partnered with a local pharmacy provider, Liberty Pharmacy, to have prescriptions delivered directly to campus for pickup in Student Health Services. Students also have the ability to have an at-home prescriber send prescriptions directly to Liberty Pharmacy for convenient delivery on campus for maintenance medications (e.g., an asthma inhaler).
How to request delivery of a prescription:
- Students must set up an account with Liberty Pharmacy (by phone) to include:
- First and last name
- Mobile number (of student)
- Home address
- Insurance information
- Name of medication
- Form of Payment
- Student’s primary care physician (PCP) will need to send the prescription directly to Liberty Pharmacy with student demographic and insurance information.
- PCP must note on script “University of New Haven Student Health” as an identifier to ensure delivery to Student Health Services.
- E-script is the best method of sending prescriptions to Liberty Pharmacy. The PCP may need the Pharmacy’s NPI: 16-89429946
- The PCP can also fax or call in the script if e-script cannot be used:
- Phone: 475-766-5454
- Fax: 475-766-5481
- Email: LibertyPharmacyCT@outlook.com
- Address:
Please note: Medications not picked up within 30 days of being delivered to campus will be discarded by Student Health Services or the Department of Consumer Protection. Student Health Services is not responsible for any costs related to any drug discarded.
